Importance of continuity of nursing care in hospital discharge of older adult patients to prevent readmissions

Introduction: The transition from hospital to home for older adults is a challenge for the healthcare system, especially due to the risk of hospital readmissions. Continuity of post-discharge nursing care is key to preventing these readmissions. Objective: To evaluate the importance of continuity of nursing care during hospital discharge in older adult patients, to identify strategies that prevent hospital readmissions and improve the quality of life of this age group. Methodology: A bibliographic review of nine articles published between 2015 and 2021, extracted from the PUBMED and EBSCO databases. Result: The most used strategies are telephone follow-up and home visits. Studies show that a lack of coordination between levels of care, poor discharge planning, and limited use of technology negatively affect continuity of care and increase readmissions. Discussion: The role of the liaison nurse, the integration of information technology platforms, management, and Orem's Self-Care Deficit Theory reinforce the importance of nursing in the transition to home care. Conclusion: Strengthening continuity of care through evidence-based strategies, advance planning, and interprofessional collaboration reduces readmissions in older adults.
