Miositis osificante traumática: Reporte de caso de una complicación poco frecuente

Abstract

Introduction: Traumatic myositis ossificans corresponds to muscle ossification caused by a traumatic injury. It is considered a rare complication, and its pathophysiology is not entirely clear. Symptoms usually include pain, stiffness, and limited movement of the affected muscle. Important risk factors for its development are severe trauma and traumatic brain injuries. Objectives: To highlight an uncommon complication in polytraumatized patients. Methodology: Clinical case analysis. Clinical case: An 18-year-old male patient was involved in a car accident without wearing a seatbelt. Upon arrival at the emergency department, initial imaging revealed various cranioencephalic injuries, pulmonary contusion, and a fractured right clavicle. He was later transferred to the Critical Care Unit, where he developed three nosocomial infections, among other complications. Two months after the accident, with stable hemodynamics and ongoing neurological recovery, he was admitted to the Intermediate Care Unit. Follow-up imaging revealed thick calcifications in both pectineus muscles and in the right iliac muscle. Physical examination of the lower limbs showed reduced hip rotation, more pronounced on the right side, as well as muscle weakness and distal hyperalgesia. Discussion: Based on the findings in the literature, it is concluded that this case of MOT exhibits the behavior typically described, both in risk factors and in clinical and imaging findings. Emphasis is placed on the need for further studies to improve its prevention and treatment.
